Here's ~stephanierain again.
I was really pleased with this one out of the camera, but I still ended up messing with it in Photoshop...

Lit with three flashes. One on camera flash as the commander shooting TTL, an SB-800, diffused camera left, and on the ground at about 1/16th and an SB-28 camera right, at about head level, at 1/8th I believe.
